Please understand the consultation is a scheduled appointment, if you cancel without a 24 hour notification you will be charged for the appointment.
If you are a new client, you must be willing to fill out and submit the following two forms.
Release From Liability Form - This form acknowledges the fact that ultimately your dog's behavior and actions are your responsibility and the Canine Behavior Center will not be held liable for any actions or behaviors your dog exhibits before, during and after any consultation or behavior modification training.
Dog Background Form - This form helps the Canine Behavior Counselor gather some very important background information about your dog.
